L'operazione sostituisce il cristallino. Ma può anche correggere difetti di vista E ntrare in sala operatoria per un intervento di cataratta. E uscirne senza avere più bisogno degli occhiali. In mezz'ora è possibile rimuovere il cristallino opacizzato, sostituirlo con una lente artificiale, e mandare a casa il paziente con minime raccomandazioni da seguire. E non solo, perché questo intervento può migliorare o risolvere disturbi di vista preesistenti anche grazie alle lenti intraoculari di ultima generazione, realizzate in materiali moderni come l'acrilico, flessibili e disponibili in diversi modelli e gradazioni. I tipi principali sono tre, pensati per risolvere differenti difetti di vista. « Le lenti monofocali sono ottimizzate per avere un solo punto di messa a fuoco, e permettono di fare a mano di occhiali per vedere da lontano in persone miopi o ipermetropi » , spiega Aldo Caporossi, responsabile dell'Unità di Oculistica del Policlinico Gemelli di Roma. Oltre a queste esistono anche le lenti toriche, che servono invece per correggere l'astigmatismo, e le multifocali che garantiscono una messa a fuoco ottimizzata per tutte le distanze. Miopia, ipermetropia e astigmatismo – assicura lo specialista – si possono correggere piuttosto facilmente nel corso dell'operazione. Mentre per la presbiopia l'unica possibilità è rappresentata dalle lenti multifocali, un'opzione che stenta a decollare per il prezzo maggiore e una serie di aberrazioni ottiche per ora ineludibili, che creano problemi di visibilità con luce artificiale o scarsa illuminazione.
« Le più comuni – continua Caporossi – sono le lenti monofocali, utilizzate nel 92 % degli interventi. Le toriche costano di più, circa 300 euro, ma permettono di risolvere l'astigmatismo che colpisce circa il 20% dei pazienti con cataratta, e di solito vengono utilizzate anche dalla sanità pubblica. Le multifocali invece costano circa 900 euro, e sforano il budget previsto dal Ssn per un intervento di cataratta » . Per questo in molte regioni non sono disponibili se non nel privato. Anche se, sul modello francese e tedesco, è possibile talvolta accedere al co-payment: ovvero la possibilità per il paziente di farsi operare in un ospedale pubblico o convenzionato pagando di tasca propria la differenza di costo.
Guardando ai numeri, le lenti intraoculari riescono a correggere i difetti refrattivi in circa il 70% dei pazienti. Ma come spiega Paolo Vinciguerra, responsabile del Centro Oculistico di Humanitas, la percentuale può crescere molto curando ogni singolo aspetto dell'intervento, anche nel periodo pre e post-operatorio. «Oggi – sottolinea – abbiamo a disposizione sistemi robotizzati che permettono di allineare la lente all'interno dell'occhio, sistemi con cui la pressione oculare viene mantenuta stabile durante tutto l'intervento, laser a femtosecondi che rendono le incisioni precisissime e ripetibili, formule che permettono di scegliere la lente più adatta per ogni paziente. Strumenti diagnostici e chirurgici che permettono un'altissima efficacia, e che bisogna saper utilizzare al meglio » . Insomma: la cataratta è diventato un intervento di precisione al altissimo impatto tecnologico, e può garantire risultati un tempo insperabili. L'équipe di Vinciguerra, ad esempio, ha recentemente sviluppato una formula matematica che permette di ottimizzare tutti gli aspetti dell'intervento: i risultati garantirebbero una visione molto soddisfacente da vicino e da lontano anche con comuni lenti intraoculari monofocali.

In addition to these there are also toric lenses, which instead serve to correct the astigmatism, and the multifocal ones that guarantee an optimized focus for all distances. Myopia, hyperopia and astigmatism - assures the specialist - can be corrected rather easily during the operation. While for presbyopia the only possibility is represented by multifocal lenses, an option that is struggling to take off for the higher price and a series of optical aberrations for now unavoidable, which create problems of visibility with artificial light or poor lighting.

Even if, on the French and German model, it is sometimes possible to access the co-payment: that is the possibility for the patient to be operated in a public or contracted hospital by paying the cost difference from his own pocket.

Looking at the numbers, the intraocular lenses are able to correct the refractive defects in about 70% of the patients. But as explained by Paolo Vinciguerra, head of the Humanitas Eye Center, the percentage can grow a lot by taking care of every single aspect of the intervention, even in the pre and post-operative period. «Today - he underlines - we have available robotic systems that make it possible to align the lens inside the eye, systems with which the ocular pressure is kept stable throughout the operation, femtosecond lasers that make precise and repeatable incisions, formulas that allow you to choose the most suitable lens for each patient. Diagnostic and surgical instruments that allow a very high efficacy, and that we must be able to use at best ». In short: the cataract has become a precision intervention with the highest technological impact, and can guarantee results that are once impossible. For example, the Vinciguerra team has recently developed a mathematical formula that allows all aspects of the intervention to be optimized: the results would guarantee a very satisfactory vision from near and far even with common monofocal intraocular lenses.
